Participation Requirements
If your student might consider trying out for one or more sports next school year, parents and athletes will have to complete all required forms online through Final Forms. You should have received an email regarding how to do this in February. If not, please contact Adam Demonbreun.
All athletes must have an updated athletic physical on file at the school to try out. Once this has been turned in to the school, it will be documented on Final Forms with the expiration date. All physicals are good for one calendar year.

Code of Conduct
Freedom Middle School would like to emphasize to all of those involved in athletic events (players, coaches, school administrators, parents and other spectators) the importance of your leadership role in sportsmanship, appropriate behavior, and integrity. The professional manner in which you conduct yourself prior to, during, and following an event, as well as any time during school hours, impacts the behavior of everyone involved.
Freedom Middle School has established guidelines for sportsmanship, citizenship, and an Athletic Code of Conduct for all to follow. Please read these carefully so we can enjoy the athletic events to the fullest and set a good example for others.
Concussion Information and Form
Public Chapter 148, effective January 1, 2014, requires that school and community organizations sponsoring youth athletic activities establish guidelines to inform and educate coaches, youth athletes and other adults involved in youth athletics about the nature, risk and symptoms of concussion/head injury.

If your daughter makes the team, we will have a mandatory parent/ cheerleader meeting on Tuesday, April 9 @ 5:30 in the FMS (Freedom Middle) media center. Be prepared to pay for practice clothes and a $100 deposit to account for us to begin registering for competitions.
At this meeting we will:
- Sign financial agreement
- Sign parent agreement
- Sign cheerleader code of conduct
- Sign up for volunteer opportunities
- Get information for our first 3 team-wide fundraisers
- Discuss the companies who have donated to us in the past and see if there are more who could
- Carpool to Premier will be discussed
- Reminder of all the fees associated with competition cheer
